  • By completing a prescription slip at the reception desk.
  • By faxing a written request to 0117 9626663 
  • By following the link on this page to the secure online form
  • By writing to us and enclosing a SAE
  • By emailing prescriptions@gp-L81622.nhs.uk

Please note we DO NOT take requests over the telephone. Prescription requests will be processed within 36 hours. If the date for your next review with a doctor or nurse has passed you will be asked to make an appointment
