There are currently 4 separate NHS initiatives regarding the way in which it is proposed that information in your medical record is held and shared with other agencies. You should have received a letter regarding the first two (2014). You have not received a letter regarding the third or fourth initiatives. Information sheets about these are available from the surgery and on our website (www.heliosbristol.co.uk) should you require further information.
1) Summary Care Record
A national system that takes a patients medication and allergies only, so that it is available if a patient is taken ill anywhere else in the country.
2) Connecting Care Record
A local system that takes the summary information from a patient’s record. This will be available to local hospitals and local Out of Hours (Brisdoc)
3) Care.Data or HSCIC (Health and Social Care Information)
This is a national system that takes identifiable patient information including chronic diseases, BMI, BP etc. into a secure central system at the HSCIC for central planning and research.
4) EMIS Search and Report
This is a reporting system which is part of EMIS. The system enables searches to be set up and run across multiple practices, and allows practices to share searches (thus reducing workload)
If you would like to opt out of any of these initiatives, please feel free to fill out an opt out form at Helios Medical Centre or click here to down load forms.
