Travel Information

If you are planning to travel abroad please contact the reception team and ask for a Travel Assessment Form. This can be collected from the surgery,emailed to you or you can download a copy from our website -follow the link at the bottom of this page to the online version.  Please return the completed form to Reception by hand, post, or fax preferably no later than 30 days before your date of travel. If we have available appointments we can offer you a consultation at less than 30 days notice. Some vaccines are free on the NHS, others require payment,which is required in full at the first visit. Either cheque or cash  - please note that currently we cannot accept debit/credit cards.

Our practice nurse will review your medical record to check which vaccines you have had previously (if recorded) and order the appropriate vaccines. One of the reception team will contact you to offer an appointment. At your appointment you will be given your vaccine(s) and health related information specific to your  travel itinery.

There will be a charge of £10 for a private prescription for such items as Malaria tablets.

WE ARE NOW A YELLOW FEVER CENTRE.   

 

As from October 2009 we are able to provide Yellow Fever vaccination to

all travellers whose destination requires it.  We can also provide this service to ANYONE, they don’t’ need to be a patient registered at the surgery.

It is important to make this initial appointment as early as possible as often a follow up appointment is required if you are receiving a course of vaccinations. Some vaccines have to be ordered as they are not a stock vaccine.  Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Most travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  This is because travel vaccination is not included in the services provided by the NHS and therefore is a chargeable service.
